In Spain, Social Security does not include lipedema surgery within its common portfolio of services. However, the possibility has opened up of funding the procedure for patients whose disease is at a very advanced stage. To qualify, strict procedures and requirements have to be met; this does not mean the operation will be funded automatically, but that the case will enter a meticulous and exhaustive assessment process by the medical committee.
What does Social Security cover in the treatment of lipedema?
Social Security has no coverage protocol for the treatment of lipedema; within its common portfolio of services it only includes medical assessment, diagnosis and some treatments. This is limited by the availability of care at the public hospital and by the criteria of each autonomous community.
Stages of lipedema and coverage criteria
Coverage of treatment, medical review and diagnosis by the Spanish public health system is not based on the results of the initial assessment, but on how far the disease has progressed and its functional impact on the patient.
The stages of the disease and the coverage criteria are as follows:
- Initial to moderate phase (stages 1-2): the skin looks smooth or has small fatty nodules that appear to be cellulite; there is tenderness or low-intensity pain. At this phase surgery is not considered necessary; the patient is referred to primary care for weight management, rehabilitation, dermatology, physiotherapy and similar.
- Advanced phase (stage 3): at this phase there are already large nodules or fat deposits causing the patient chronic pain and even affecting their mobility. Public health staff prescribe therapeutic treatment such as lymphatic drainage, rehabilitation, physiotherapy and self-care education.
- Critical phase (stage 4): at this phase the lymphatic system has already collapsed and lipolymphoedema is present. Public healthcare therefore extends its coverage and includes compression garments and physiotherapy sessions within the conservative treatment.
When does Social Security actually cover the surgery?
The National Institute of Social Security covers lipedema surgery when it is shown that the patient suffers severe mobility limitation preventing them from carrying out basic activities such as personal hygiene, when swelling and chronic pain have been documented and it is shown that they no longer respond to painkiller treatments.
Other factors that influence the decision to authorise surgery are: the failure of conservative treatment, which consists of compression garments, lymphatic drainage, rehabilitation therapy, diet and so on.
And finally, approval is required from the medical tribunal certifying that the surgery is reconstructive and a last resort, not cosmetic.

Real waiting times in public healthcare
Because there is no protocol for performing lipedema surgery in Spanish public health legislation, there is no established waiting time. In the exceptional cases where patients have managed to get the procedure approved, the process has taken an average of one to three years.
Functional versus cosmetic surgery: why coverage changes
Functional lipedema surgery and cosmetic lipedema surgery have different aims. The first focuses on improving the patient’s health and the second on improving their physical appearance. That is where coverage changes, since public healthcare does not fund surgery for cosmetic purposes.
What is functional surgery in lipedema?
Functional or reconstructive lipedema surgery is surgery whose main aim is to restore the patient’s health and give them back their quality of life. It consists of removing the diseased adipose tissue that causes them chronic pain, immobility, problems in the lymphatic system and so on. It is not an operation intended to improve physical or outward appearance.
What is cosmetic surgery in lipedema?
Cosmetic lipedema surgery focuses on removing deformities, improving the contour of the area, refining the silhouette and correcting body asymmetries. At this phase the patient’s health is not compromised by the condition, and it is carried out in the early stages or phases of the pathology.
What to ask for at your appointment to qualify for public coverage
To meet the requirements demanded by Spanish public healthcare in order to qualify for coverage of lipedema surgery, you need to request the following: a medical report in which the diagnosis is clearly set out; it must include details of the type and stage of the disease and its consequences.
Evidence and certification that conservative treatment has failed (compression garments, lymphatic drainage and so on). Diagnostic tests such as a Doppler ultrasound and a lymphophlebography and, finally, priority referral to the Plastic and Reconstructive Surgery Department of your hospital. They are the ones responsible for taking the case before the medical committee to assess and decide on funding the operation.
Going private: guide price for lipedema surgery
Given the long waiting periods and how cumbersome the bureaucratic paperwork is that a patient has to complete for public healthcare to assess their case and decide whether or not the surgery will be funded, there is another alternative. This option consists of having the procedure done privately, at your own expense, in a private clinic.
How much does lipedema surgery cost in a private clinic?
In private clinics in Spain, the guide price for lipedema surgery sits between €5,500 and €6,100 per session. These prices can vary because there are factors involved in calculating them, such as how specialised the clinic is, how far the disease has progressed, the location, the surgeon’s reputation and so on.
Financing options for lipedema surgery
Since the option of treatment through public healthcare is so limited, there are other options a patient can consider in order to have the surgery; among them are:
- Financing from the clinic where the operation is to be performed: there are specialist clinics that offer this service in instalments or on flexible terms.
- Bank loans: some banks include health consumer loans in their lending portfolio.
- Private health insurance: some health policies cover lipedema surgery for medical, not cosmetic, purposes, generally on a reimbursement basis and once the case has been evidenced.

Does Social Security operate on lipedema in Spain?
Although it is not within the public health portfolio of services, there is a possibility that Social Security will fund the surgery in exceptional cases, provided a strict, rigorous and lengthy medical and bureaucratic procedure is completed.
