Any other questions about bichectomy?

First, local anesthesia is applied to the inside of the cheeks, inside the mouth, then a small incision is made. Through it, the Bichat’s fat pads are removed and closed with a resorbable suture that doesn’t need to be removed at the clinic. The stitches fall out on their own.

In 3D bichectomy, in addition to this, the fat in the jawline and under the chin is removed to define the jawline through liposculpting.

It’s also possible to perform a cheekbone lipofilling with the extracted fatty tissue to further define facial features.

They are usually visible at 2-3 months, although it depends on each patient.

They are fatty tissue located in the cheeks, below the cheekbones. When we’re babies they’re more pronounced, tending to diminish as we grow. As adults, they remain more or less large depending on each person’s genetics. Here, diet and exercise have little to do with it.

No, they don’t grow back once they’re removed. If you gain weight, you may recover some facial roundness due to an increase in subcutaneous fat, not from the growth of Bichat’s fat pads.

For all those people with a healthy weight who have a round face and can’t slim down through diet or exercise. When there’s excess weight, it’s not advised, as the effects are barely noticeable.

Patients with excessive facial sagging or advanced age should be evaluated carefully by the surgeon. Most of the time, combining it with a facelift brings better results than bichectomy alone.

It’s recommended especially between 18 and 45 years old, since the face is fully formed and won’t undergo significant changes. Performing it later, when the face already tends to lose fat, could result in an undesired outcome.

In any case, it’s essential that the doctor evaluates whether you’re a candidate for surgery at your free initial consultation.

Bichectomy is performed under local anesthesia, so you won’t feel anything. It’s painless.

It’s quite comfortable, since you won’t even need to wear compression garments like girdles or chin straps. You’ll simply need to follow these guidelines:

  • Avoid alcohol and tobacco.
  • For a few days, it’s advisable to follow a soft diet to avoid straining your chewing muscles. During the first 24 hours after surgery, you’ll consume only liquids.
  • It will be important to maintain good oral hygiene. This will help prevent infections.
  • You can apply cold compresses or ice to your cheeks to relieve swelling.

The doctor will provide all post-operative instructions following your surgery.

You’ll also be scheduled for follow-up visits at the clinic at 15 days and at 3 months to ensure everything is progressing beautifully.

You’ll be able to return to your daily life 24 hours after the procedure, although it’s recommended that you avoid strenuous physical activity for at least 2 weeks afterward. As for work, you may prefer to take a few days off if your cheeks become swollen.

During this initial consultation, the doctor will analyze your desires and expectations. He will evaluate your Bichat’s buccal fat pads and will be honest with you about whether the treatment is right for you.

If it is, he will explain the pre-operative requirements, the risks associated with the surgery, and what you need to do to avoid them. Additionally, he will show you before and after photos from other patients with a similar concern to yours so you can realistically see what results you can achieve.

Of course, to ensure there’s nothing that would contraindicate bichectomy, he will also review your medical history: medical conditions, medications, allergies, etc.
